Pawhuska Family Medical Clinic is now offering Chronic Care Management (CCM) as a way of improving the lives and mitigating hospitalizations for those suffering with chronic diseases. Our CCM program provides patients with a dedicated healthcare team who will curate a customized care plan to help them:

  • Cope with disease symptoms
  • Manage medications
  • Reduce number of hospitalizations and ER visits

Chronic Diseases

A chronic disease is an illness that lasts a year or more, requires ongoing medical attention, and may limit activities of daily life. Our Chronic Care Management program helps those suffering from illnesses, such as: 

  • Alzheimer’s disease and related dementia
  • Arthritis (osteoarthritis and rheumatoid)
  • Asthma
  • Atrial Fibrillation
  • Autism spectrum disorders
  • Cancer
  • COPD
  • Depression
  • Diabetes
  • Heart Failure
  • Hypertension
  • Ischemic heart disease
  • Osteoporosis

What to Expect

Patients who take advantage of our CCM program will benefit from a personalized care program specific to  their individual needs and health management goals, a dedicated healthcare team who understands all the struggles of chronic illnesses, and a support system to provide counsel and encouragement in trying times. 

Patients enrolled in this program will receive monthly “checkups” via phone or email (their preference) from a member of their healthcare team. This allows them to discuss symptoms, progress, medications, challenges, and steps taken to alleviate problems brought on by diseases. 

Chronic Care Management services are covered by insurance and subject to copays. Once enrolled, patients may choose to stop receiving CCM services at any time.
